VNA’s Personal Navigator Program Division Expands Services to Phoenixville, PA!

VNA Community Services Personal Navigator Program Expands Services to the Greater Phoenixville, PA Area!

We are pleased to share that a neighborhood grant is allowing us to expand our services into the Greater Phoenixville area!

Did you know that $30 billion in critical benefit dollars, such as SNAP, utility, and prescription help are left on the table each year across America? Let’s make sure those in need across our community have access to these dollars that can help lift them out of poverty.

The Personal Navigator Program serves Montgomery County and Phoenixville area residents through the lifespan in connection and enrollment to benefits that improve health, economic, and food security.  Simply put, we are a one-stop shop for public benefits screenings, enrollment, and acquisition.

We are a free community-funded program that is designed to connect eligible individuals to benefits that include:

  • CHIP
  • Medical Assistance
  • Health Insurance Marketplace (Pennie)
  • Medicare-related benefits and subsidies
  • Free or low-cost medication
  • PACE/PACENET
  • SNAP
  • LIHEAP/LIWAP
  • PA Property Tax/Rent Rebate
  • Affordable Connectivity Program
  • Free Cell Phone Program
  • Other financial and social supports

Since 2005, the VNA Navigator program has connected over 26,000 Montgomery County families to $40M in cumulative benefits to help families pay for groceries, and healthcare costs and to heat their homes.

We are so excited to be bringing these critical services to the Phoenixville area, if you or a loved one are interested in more information, please connect with us by phone or web. To make a  referral, please contact our intake staff at 1-800-591-8234 or complete an online enrollment through our secure online enrollment portal.
