Connecting Montgomery County to Lifeline Benefits

Since 2005, we’ve proudly operated as a free, community-funded program connecting eligible Montgomery County residents to services that improve health, economic stability, and food security.

SSI/SSDI Outreach, Access, and Recovery (SOAR)

SOAR is a free, national program designed to increase access to Social Security disability benefits for individuals who are experiencing or at risk of homelessness and living with a serious mental illness, co-occurring substance use disorder, or medical impairment.

VNA’s SOAR program provides intensive support to help eligible individuals apply for and receive SSI/SSDI benefits. It simplifies a complex process and delivers real outcomes.

We serve Montgomery County in partnership with Your Way Home and with support from the Department of Health & Human Services.
📈 Proven Results:

VNA’s SOAR Program uses an expedited application model with an 85% approval rate in under 70 days for qualifying individuals— compared to the national average of 65% approval.

These benefits open doors to housing, treatment and income for people that need it most.
